A Power of Attorney is a legal document that you may use to authorize another individual to act on your behalf. There are different forms that provide various powers to the named individual.

Health Issues
A Durable Power of Attorney for Health Care is a legal document that designates a person or persons to make health care decisions on your behalf should you become unable to make those decisions yourself. This person or persons act as your agent and should be someone you trust. This office creates documents which will become effective when it is determined by a physician that you have become disabled or incapacitated and at that point your agent takes over the decisions involving your health care.
